Apply Online For Individual or Marketplace Health, Dental, and Vision Insurance

There are two ways for Brooks Financial to assist you with purchasing individual health, dental, and vision insurance:

Option 1: If you are not eligible for a government subsidy, purchase coverage directly from a carrier using a Brooks Financial-specific carrier link below.

This option provides more plan choices and flexibility.

  • Click the carrier link below to access a Brooks Financial-specific carrier website.

  • From there, you can run a quote and apply for coverage directly.

⚠️ Important: If you visit a carrier’s website outside of our Brooks Financial-specific link, Brooks Financial will not be assigned as your agent and we will be unable to assist you with your policy.

Option 2: If you think you are eligible for a government subsidy, purchase through the Health Insurance Marketplace with Brooks Financial as your agent.

You will need to visit www.healthcare.gov and create and account.

  • Complete the online application to check subsidy eligibility.

  • When prompted, enter:

    Agent Name: Jason E. Brooks

    FFM/NPN #: 538707

    This ensures Brooks Financial is assigned as the agent to your policy so we can support you with plan selection, enrollment assistance, claims problems, and billing issues.

  • Once you receive your subsidy determination, we can help you choose the plan that best fits your needs.
